Mpox Classified as Class B Infectious Disease with Corresponding Control Measures from September 20

According to the National Health Commission (NHC) official website, in accordance with the provisions of the People's Republic of China Law on the Prevention and Control of Infectious Diseases, mpox has been classified as a Class B infectious disease starting from September 20, 2023, with corresponding prevention and control measures implemented.






Mpox is a zoonotic disease caused by Mpox virus (MPXV) infection. In September 2022, China reported its first imported mpox case. Since June 2023, multiple provinces across China have successively reported numerous mpox cases, posing a high risk of local secondary outbreaks and hidden transmission.




Over 1,000 Local Cases Reported

Mpox Epidemic Will Persist in China for a Certain Period

According to data released by the Chinese Center for Disease Control and Prevention (China CDC), 106, 491, and 501 new confirmed mpox cases were reported in June, July, and August respectively, with the total number of local mpox cases exceeding 1,000 within three months.




Key Epidemic Characteristics at Current Stage
Ø The vast majority of cases are among Men who have Sex with Men (MSM)
Ø Female cases have been reported, mostly likely infected through heterosexual contact
Ø Low risk for the general population; low transmission risk through other contact routes
Ø Most cases are identified through voluntary medical consultation and close contact tracing
Ø Some mpox patients present with atypical clinical manifestations

Issued by the National Health Commission and the National Administration of Disease Prevention and Control on July 26, 2023 (Guo Ji Kong Chuan Fang Fa 〔2023〕 No. 16).
Expert Consensus on Hospital Infection Prevention and Control of Mpox
Initiated jointly by the National Healthcare Institution Infection Surveillance Network, the National Training Base for Hospital Infection Control and Management, and the National Clinical Research Center for Geriatric Diseases, published in the Chinese Medical Journal, Volume 103, Issue 34, September 2023.

High-Risk Groups 
MSM: MSM has been the main affected population in mpox outbreaks in many countries worldwide. Transmission occurs through sexual contact, spreading via large gatherings and subsequent community transmission among MSM to multiple countries and regions globally.
Healthcare Workers: Survey results show that 70% of medical personnel did not take appropriate personal protective measures when first encountering human mpox cases.

Multi-Channel Surveillance
1. Healthcare Institution Surveillance
Medical institutions at all levels and types shall conduct surveillance for mpox-like symptoms, with particular attention to patients with such symptoms in departments including Dermatology (Venereology), Anorectal Surgery, and AIDS Voluntary Counseling and Testing Clinics.
2. Key Population Surveillance
Medical and CDC institutions shall proactively inquire about mpox-like symptoms and suspected exposure history when providing diagnosis, treatment, or testing consultation services to key populations such as MSM.
3. Entry-Exit Personnel Surveillance
Customs authorities shall promptly collect specimens and conduct nucleic acid testing for entry-exit personnel who make abnormal health declarations related to mpox or are found to have mpox-like symptoms during quarantine.
4. Sewage Surveillance in Key Venues
Based on the epidemic situation, conduct sewage surveillance in key venues frequently visited by MSM, such as bars, clubs, and bathhouses.

WHO-Recommended Nucleic Acid Detection Methods
The WHO recommends real-time PCR and/or sequencing as confirmatory methods for mpox virus. It is recommended to prioritize the use of mpox virus-specific real-time PCR or orthopoxvirus pan-detection methods for testing suspected samples.

For mpox virus nucleic acid detection, specimens from skin or mucosal lesions are preferred, and oropharyngeal swabs can be collected simultaneously.
